Complete Your New Patient Forms
Before we schedule an evaluation, we’ll need some information about your child, their history, and your concerns. You can complete the necessary forms below and return them to our team.
We’ll Verify Your Insurance
Once we receive your completed forms, our team will verify your insurance coverage. Buford Kids’ SPOT currently accepts:
- Aetna
- Amerigroup
- Anthem
- Blue Cross Blue Shield
- CareSource
- Peach State and Peach Care
- TRICARE
- UnitedHealthcare

Schedule Your Child’s Evaluation
Once the intake and insurance verification process is complete, we’ll schedule your child’s evaluation. Evaluations typically include standardized testing; when standardized testing isn’t appropriate for a child, our therapists can use an observational, play-based approach instead.
After the evaluation, we’ll discuss the next steps and, when therapy is recommended, establish your child’s ongoing appointment schedule.
